Welcome to Hudson Valley Community College Health Services!

Health Services offers responsive, accessible, individualized health care for all students while on campus.

Students can be seen on a walk-in basis during office hours. Feel free to stop in to make an appointment with the nurse practitioner or the college physician.

FAQs

Q: Are vaccines and immunizations required?
A: All students must submit proof of immunizations to healthservice@hvcc.edu or fax to (518) 629-7471.

Required Vaccines:

  • MMR 2 (Measles, Mumps, and Rubella Virus Vaccine Live) for those taking six or more credits on campus
  • All students must complete a Meningitis Self Reporting Form (PDF). Proof of vaccination within five years can be submitted to Health Services.

Most students can request their records from their high school or prior college, doctor’s office, or place of employment. Students may also obtain MMR titers (bloodwork that tests for antibodies) from their local doctor’s office or urgent care if they do not have access to their MMR records

Proof of COVID-19 Vaccines can be uploaded to the COVID-19 Vaccination Documentation Form.

Q. May I apply for a religious or medical exemption to the COVID-19 vaccine?
A. An exemptions form is required for any student seeking to submit a medical and/or religious waiver from NYS immunization and vaccination requirements.

Q: I tested positive for COVID and/or have symptoms of COVID-19. What do I do?
A: Stay home and begin isolation immediately. Call (518) 629-7468 and ask to speak with a nurse or email heatlhservice@hvcc.edu. Submit your test result at this link — COVID-19 test result.

Q: I finished my isolation. When can I come back to campus?
A: To gain clearance to return to campus, students must request a communication from a college health nurse at (518) 629-7468 or healthservice@hvcc.edu. Your release will be sent in your HVCC student email account.

If you are transferring to another school and need immunization records, please submit an Authorization for Disclosure Form (PDF) to healthservice@hvcc.edu. We will gladly send your records to your new school.


COVID-19 Testing on Campus

Weekly pooled surveillance testing is available to all students and employees, however, weekly testing is required for all unvaccinated or partially vaccinated students, including those with an approved exemption who are on campus.

Students may also meet the weekly testing requirement by submitting results of an at-home test to Health Services using the  Documentation of COVID-19 Test Results form.
NOTE: Test kits are available at no cost at Health Services, located in the Siek Campus Center, Room 270, and TEC-SMART during normal business hours and also at the Center for Access and Assistive Technology until 6 p.m.

The Rensselaer County Department of Health offers a COVID-19 Vaccine and Booster Clinic from 3 to 6 p.m. every Wednesday in the McDonough Sports Complex.
